| To register
a student in Frederick County Public Schools, the legal parent/guardian
must enroll the student and provide:
1.
Proof of Residency - supply one of the following:
Current
coy of rental lease or contract
Current copy of property tax bill
Current copy of utility bill with parent/guardian's name and
address (phone, TV, gas, electric, or water)
If you live with someone else, the owner/lessee needs to provide
a notarized affidavit verifying that the child resides at
that address, plus provide proof of his or her residence.
2.
Proof of Birth - supply one of the following:
Certificate
of Birth
Birth Certificate
Passport
Hospital certificate
3.
Social Security
Evidence
of United States federal social security is requested for
United States
citizens and non-citizens who hold documentation of permanent
residency.
4.
Health Documentation
Documentation
of proper immunizations or appointment to obtain missing shots
Transfers from out of state are required to have a physical
within six months of enrollment
Medication taken during the school day must have proper paperwork
in the health room
5.
School Records
Transfer
grades
Most recent report cards
Special Education or 504-Plan papers (if applicable)
6.
Custody/Court Documents (if applicable)
