Before medication
can be administered in school by health room or
school staff, a
FCPS Medication Authorization Form must be
completed by the student’s doctor and signed by
parent/guardian. Information from a doctor on a
prescription pad does not have all the
information necessary to ensure safe medication
administration in school.
Prescription
medication (including inhalers) must be in a
container labeled by the pharmacist.
Over –the
–counter medication must be in the original
unopened container.
Students are
prohibited from transporting medications.
If you are
anticipating that your child may need to take
medication during the school day, please stop by
the health room to pick up the form, as doctor
offices and emergency rooms do not stock the
