All medications must be in the original prescription container.

medication will not be given in school.
The parent/guardian is responsible for submitting a new form
to the school each time there is a change in dosage or time
of administration.
Immunizations
Each student must have an up to date immunization record
prior to entering school for the first time.
Emergency Cards
Each student should have an up to date emergency card completed
with at least one emergency contact number in the event that
the parents/guardians are unreachable in case of an emergency.
Illness
The following are some guidelines on when to keep your child
home from school in case of illness:
1. A temperature of 100 degrees or above anytime in the past
24 hours
2. Vomiting or diarrhea anytime in the past 24 hours
3. Pink Eye: The child may not return to school until he/she
has been seen by a doctor and on medication for at least 24
hours. A doctor’s note is also needed stating the child
is no longer contagious.
4. Strep Throat: The child may not return to school until
he/she has been on an antibiotic for at least 24 hours and
no longer running a fever.
5. Ringworm: The child may not return to school until he/she
has been on medication for at least 24 hours. Also, a doctor’s
note is needed stating the child may return to school.

at by a physician.
Fewer illnesses/communicable diseases will be “shared”
between classmates if these guidelines are followed.
