General Registration Requirements
To register a student in Fredrick County Public Schools, the legal parent/guardian must enroll the student and provide:
1. Proof of Residency - supply one of the following:
- Current coy of rental lease or contract
- Current copy of property tax bill
- Current copy of utility bill with parent/guardian's name and address (phone, TV,
gas, electric, or water)
- If you live with someone else, the owner/lessee needs to provide a notarized
affidavit verifying that the child resides at that address, plus provide proof
of his or her residence.
2. Proof of Birth - supply one of the following:
- Certificate of Birth
- Birth Certificate
- Passport
- Hospital certificate
3. Social Security
- Evidence of United States federal social security is requested for United States
citizens and non-citizens who hold documentation of permanent residency.
4. Health Documentation
- Documentation of proper immunizations or appointment to obtain missing shots
- Transfers from out of state are required to have a physical within six months of enrollment
- Medication taken during the school day must have proper paperwork in the
health room
5. School Records
- Transfer grades
- Most recent report cards
- Special Education or 504-Plan papers (if applicable)
6. Custody/Court Documents (if applicable)
