The BMS PTO is publishing a student directory for the 2009-2010 school year. The student directory is intended to be a helpful family resource. To be included in the directory, this form must be filled out and signed by a parent/guardian. Every BMS family will receive a copy. Please return this form to your advisory teacher. This directory will go out to over 600 local households which is a great way to advertise in your community!

FULL PAGE REQUIREMENTS:

· Ads should have ½ inch margins top and bottom, left and right.

· Based on a standard 8 ½ x 11 inch paper.

· Full is done in portrait

HALF PAGE REQUIREMENTS:

· Ads should have ½ inch margins top and bottom, left and right.

· ½ page is landscape

· ½ margins based on 4 x 5 ½
